General Information

New Patient Form

Please complete our New Patient Form before you arrive so we can spend your appointment time treating you.  When you book your appointment you will receive a confirmation email with a link to enter the required information online.  Alternatively, click HERE to download and print a copy of the form to complete and bring with you to your appointment.

Podiatry Referral

Please ensure ALL referrals are completed at time of booking appointment. We ask all referrals to be written to Boots and All Podiatry and received at the nominated clinic prior to attending your appointment. Please Note:  Podiatry time taken to obtain referral will be considered part of your appointment time. 

Clients who have a current Medicare referral from their doctor will need to be prepared to pay an upfront fee on the day of their appointment. It is the client’s responsibility to ensure the required documentation is provided to be eligible for claiming of their Medicare rebate, and banking details are registered with Medicare to receive rebate.

Attending Appointments

If you are unwell, or experiencing respiratory symptoms of COVID-19 or you are deemed a close contact of a person with COVID 19, pending test results, please DO NOT attend your appointment.

When attending your appointment, please wear fitted face mask, sanitise hands and allow physical distancing. Please advise your clinician of any medical illness, medications, or changes in medical health.

Arrival Time

Please arrival 10 mins prior to your appointment time. If you arrive late, please be aware your treatment time may be cut short and the full price of the treatment will be charged.

Clothing and Footwear

Please wear practical clothing and bring your footwear most often worn and orthotics/ in-shoe supports you may have.

Children

We prefer and encourage children to be minded during your visit. If children need to attend appointment with client, they need to be always supervised. Clinicians may cease consultation if disrupted or have safety concerns.

Mobile Devices

Please turn off/switch to silent mobile phones and devices while in consultation.

Cancellations

If needing to cancel your appointment, please allow at least 24 hours notice if you are unable to attend. A cancellation fee is charged.

Pricing & Payments

Pricing is subject to change without notice. Payment is expected at the time of treatment, we accept cash, eftpos and cheque. Private Health with ancillaries can often be claimed at the time of payment, if health funds are compatible with practice equipment, otherwise client will need to contact their health fund for claiming.
